If you are a professional who has contact with Carers in the Preston area we would welcome your referrals.
These
can be made:

- by post, or in person, to

Preston PR1 3BQ
We will need the following information from you:
- Carers Name, Address and contact phone number
- Confirmation that you have obtained permission from the Carer for you to pass their details on to us
- Your details i.e. where you work and a direct line for us to contact you if we have any queries
- Reason for the referral.
